New Student Registration

Welcome to the Clinton Township School District!

 It is important for families new to the district, or with children entering school for the first time, to register as early as possible so we can schedule classroom assignments and bus routes. Pre-registration is an option if your child will be entering the district during the school year.

The following records/forms need to be presented at the time of registration:

  • Birth Certificate ~ ORIGINAL - with raised seal
  • Proof Of Residence

One of the following may be accepted as proof of residence:

  • Purchase Agreement/Deed
  • Property Tax Bill/Document
  • Mortgage Statement
  • Lease Agreement


  • Registration Form
  • Application for Student Transportation Form
  • Emergency Care Form
  • Health History & Physical Examination Forms (to be completed by Parent AND Physician)
  • Immunization Record
  • Acceptable Use of Technology Form
  • Publicity Consent Form
  • Childhood History Form
  • Home Language Survey
  • Custody/Guardianship papers (if applicable)

All students entering the district must provide proof of a current physical, (within the last year), and up-to-date immunizations.

If your student is entering from another school or homeschooling, please complete this record release form and return with your supporting documents.

If your child is entering kindergarten through grade 5, please complete this packet:

If your child is entering grades 6-8, please complete this packet:
6-8th Grade CTSD Registration Packet
Please note: Pre-participation physical evaluation forms are for 6th graders ONLY.

All of the forms required are in the above packets. Having these forms filled out ahead of time will significantly speed up the registration process.

Option 2: Online Student Registration

We are now able to offer online new student registration. While most of the registration process can be completed online, we require a paper copy of some of the required registration documents. The specific forms are indicated in the online registration system.

Does your child have health insurance? If not, please visit It is a law in NJ that all children age 18 and under MUST have health insurance. NJ Family Care is a federal and state funded health insurance program, which provides free or low cost insurance for parents and children.
